On AI, Oncology & Equity — A Physician’s Perspective on Smarter Care Without Losing the Human Context

Let me tell you what a rural radiation oncology shortage actually looks like from the inside.

Not the policy brief version. Not the workforce study footnote version.

I am a board-certified radiation oncologist. I am also a retired U.S. Army Lieutenant Colonel who began her military career as an enlisted Soldier. And I am a veteran — which means when I talk about what rural veterans face in accessing cancer care, I am not speaking from the outside looking in. I have been inside that system. I understand the culture, the language, the loyalty, and the particular frustration of a population that gave everything and is now navigating a VA system that cannot always meet them where they are.

That triple perspective — oncologist, veteran, VA advocate — is what I brought to the American College of Radiology (ACR) Annual Meeting in April 2024, and it is what I bring to this conversation now.

Here is what this shortage actually looks like: a patient who drives 90 minutes each way, five days a week, for six weeks, to receive radiation treatment — because the nearest clinic is the nearest clinic. A veteran who was told telehealth was available, but whose home internet cannot support a video call. A position posted for eighteen months with no viable candidate, and a community quietly, without announcement, losing access to cancer care.

In April 2024, I presented on exactly this at the American College of Radiology (ACR) Annual Meeting — a national platform where I laid out the case for how AI could augment the radiation oncology workforce in rural communities, address the broadband gap blocking veteran access to telehealth, and support remote clinical oversight in areas that simply cannot recruit a full-time physician. I was not speaking theoretically. I have opened two radiation oncology clinics — one academic, one private practice — and I have watched these gaps operate in real time, from the inside.

I share that context not to establish credentials, but to be clear about where this perspective comes from. This is not an AI enthusiast’s take on medicine. This is a physician’s take on AI — grounded in what the clinical reality actually looks like for the patients most at risk of falling through.

The Workforce Math Does Not Work

The ASTRO Workforce Study documented what many of us already knew: approximately 85–90% of radiation oncologists practice in metropolitan areas, leaving rural regions—home to about 15–20% of the U.S. population—relatively underserved. A substantial proportion of the current rural workforce is approaching retirement, with some surveys suggesting up to one-third plan to retire within the next decade.

We are not replacing them fast enough. Radiation oncology training programs are not geographically redistributing graduates into underserved communities at the rate needed. Financial incentives, professional isolation, quality of life concerns, and infrastructure gaps make rural practice a hard sell for newly trained physicians — however much we might wish otherwise.

This is not a pipeline failure alone. It is a distribution failure. And it has real consequences for real patients right now, not in some projected future.

What AI Can Actually Do

I want to be careful here, because this is where the conversation tends to go off the rails in one of two directions — either AI is going to solve everything, or AI is a threat to clinical standards. Neither is accurate.

At the ACR Annual Meeting, I walked through specific AI applications in radiation oncology — not as future possibilities, but as tools already operating in clinical settings. What follows is drawn from that presentation and from my ongoing work in this space. I offer it as a physician accounting for what I have actually seen, not what a vendor has promised.

Here is what AI can genuinely do for rural radiation oncology access today:

Automated contouring.

One of the most time-intensive tasks in radiation oncology is delineating — manually drawing — the boundaries of a tumor and surrounding critical structures on imaging. AI-assisted auto-contouring can reduce this from hours to minutes, enabling a single physician to support more patients across more sites without sacrificing accuracy. For rural clinics that rely on remote physician oversight, this is not a convenience. It is a capacity multiplier.

Adaptive radiation therapy support.

As a patient’s anatomy changes during treatment — which it does — the radiation plan needs to adjust. AI-assisted adaptive planning enables this to happen with greater speed and precision than manual replanning allows. This is particularly meaningful in settings where the physician is not physically present every day.

Remote treatment planning review.

A board-certified radiation oncologist can review and approve AI-assisted treatment plans from a distance. This model — already operating in various forms across health systems — allows clinical expertise to reach communities that cannot recruit a full-time physician. Done with appropriate protocols and oversight, it is not a compromise of care. It is an extension of it.

Quality assurance and error flagging.

The Radiation Oncology Incident Learning System’s (RO-ILS) analyses should give every clinician pause: RO-ILS have consistently shown that a significant proportion of errors are first detected by radiation therapists at the linear accelerator — at the machine, in real time. AI-assisted QA tools that flag anomalies before a plan reaches the patient add a layer of safety that is especially critical when fewer eyes are on each case.

What AI Cannot Fix

I want to be equally clear about the limits, because this is where honest conversation matters more than optimism.

AI cannot recruit a physician to a rural community. It cannot address the broadband gap that prevents 25–40% of rural and highly rural veterans from accessing VA telehealth from their own homes. It cannot resolve the financial and regulatory structures that make rural practice unsustainable for many physicians. And it cannot replace the clinical judgment that comes from a physician who knows the patient, has reviewed the full record, and is accountable for the decision.

I want you to sit with that 25–40% figure for a moment, because as a veteran I find it unacceptable in a way that goes beyond statistics. We ask men and women to serve in some of the most remote, resource-scarce environments on earth. We thank them for their service. And then we design a telehealth system that requires broadband internet — and are surprised when nearly half of rural veterans can’t access it from home. That is not a technology failure. That is a planning failure. And it is one that AI alone cannot fix, but that AI-assisted mobile care, remote consultation models, and community-based access points can meaningfully begin to address — if we commit to building them with the veteran’s actual situation in mind, not the ideal one.

The supervision question is real. During the COVID-19 pandemic, virtual supervision of radiation services was permitted — and in many cases, it worked. The subsequent push to revert to mandatory in-person supervision for all radiation services came from legitimate patient safety concerns. I respect that. But the question the field must sit with is this: what is the patient safety impact of no radiation oncologist at all? For communities where the answer to that question is increasingly “that is our reality,” AI-assisted remote oversight — with rigorous protocols, documented accountability, and physician-in-the-loop at every critical decision point — is not a workaround. It is a model worth building seriously.

The Question I Keep Coming Back To

I am not an AI vendor. I am not a technologist. I am a radiation oncologist who has spent her career watching what happens when access is absent — in underserved communities, in rural clinics, in VA waiting rooms, and in cancer centers that serve patients who have already waited too long.

The question I keep coming back to is not “can AI do this?” The technology is capable of remarkable things. The question is: “will we deploy it with the patient who needs it most at the center of that decision?”

That is a human question. A leadership question. A values question.

AI is a tool. A powerful one. But tools do not make decisions about who deserves access to cancer care.

We do.

On Service, Exposure & Evidence — An Oncologist Veteran-to-Veteran Perspective

If your VA bladder cancer claim was denied — or you have not filed yet because you did not believe you would qualify — please stop what you are doing and read this carefully.

Bladder cancer is an important consideration in veteran health, particularly when toxic exposure may have played a role in its development. Many veterans and families are uncertain whether they qualify for VA benefits — especially when a claim has been denied or never filed.

As a physician who has reviewed cases like these, I often see situations where the relationship between a diagnosis and a service history was never fully developed — even when the exposure record suggests a possible pathway. This article reviews the main routes that may apply and explains the medical evidence that often matters most.

Agent Orange and Presumptive Service Connection

The first thing most veterans do not know:

Bladder cancer is recognized by the Department of Veterans Affairs as a presumptive condition associated with herbicide exposure, including Agent Orange. [VA source]

Veterans who served in qualifying locations — including Vietnam, certain areas of the Korean DMZ, Thailand bases, and other recognized exposure settings — may qualify for presumptive service connection.

Under a presumptive framework, the VA accepts the recognized association between exposure and disease without requiring the veteran to independently prove causation.

Presumptive status reduces the evidentiary burden — but it does not make the process automatic. Incomplete records or unclear documentation can still lead to delays or errors in how a claim is adjudicated/processed.

In other words, it means the burden of proof shifts — but you still have to file correctly. You still have to document your service location. You still have to make sure the VA actually sees and records the connection between your diagnosis and your service history.

Filing without guidance is one of the most common reasons these claims get denied or likely assigned a low rating — not because the evidence was not there, but because no one organized it properly.

Camp Lejeune Water Contamination

Individuals who lived or worked at Marine Corps Base Camp Lejeune between 1953 and 1987 were exposed to contaminated drinking water containing compounds including trichloroethylene, tetrachloroethylene, benzene, and vinyl chloride — substances well documented as carcinogenic.

The VA recognizes bladder cancer as a presumptive condition for Camp Lejeune-related exposure. Bladder cancer is recognized as a Tier 1 condition under the Camp Lejeune Justice Act — the highest level of recognized connection between that exposure and a bladder cancer diagnosis. When Your Bladder Cancer Is Non-Presumptive — And What That Actually Means

Not every veteran fits neatly into the Agent Orange, Camp Lejeune, or other PACT Act presumptive diagnosis categories. Maybe you were exposed to burn pits. Maybe you handled industrial chemicals or solvents during your service. Maybe you were stationed somewhere the VA has not yet officially recognized.

The 2022 PACT Act significantly expanded toxic exposure coverage and added new presumptive conditions (check the list regularly for updates). Many veterans who believe they have a non-presumptive situation actually qualify for a presumption now — they simply have not been told.

The Role of Medical Nexus Evidence

For non-presumptive claims, medical evidence becomes the foundation of the case.

The VA generally looks for three elements:

1. A current diagnosis.

2. Evidence of an in-service event, exposure, or illness.

3. A medical nexus linking the condition to service.

A nexus opinion is a formal medical assessment that addresses whether your condition is related to your military service. Strong opinions typically include a review of service records, medical records, and relevant exposure history, along with clear clinical reasoning grounded in the applicable literature and research.

In some cases, additional pathways may apply — including secondary service connection or aggravation of a pre-existing condition. A medical record review can help identify what is present in a file and what may need to be developed further.

Considerations for Surviving Family Members

For surviving spouses and dependents, Dependency and Indemnity Compensation (DIC) may be available if a veteran’s death is connected to service.

DIC may apply when bladder cancer was already service-connected, when a connection could have been established with stronger evidence, or when the condition contributed to the veteran’s death. Even when a veteran did not file or complete a claim during their lifetime, a review of the service and medical record may still identify a possible pathway.

Moving Forward

Bladder cancer claims — whether presumptive or non-presumptive — often depend on how clearly the medical and service evidence are presented and documented.

A structured review may help clarify whether:

• A presumptive pathway applies.

• Additional medical evidence is needed.

• A prior denial may warrant another look.

• A survivor claim may still be open.

Even if service connection was never formally established before the veteran’s passing, there may still be options worth exploring.

Does the PACT Act cover bladder cancer?

The 2022 PACT Act significantly expanded presumptive conditions and toxic exposure coverage. I get asked a version of this question more often than any other right now:

“Is AI going to replace physicians?”

Sometimes it comes from medical students nervous about their career choice. Sometimes from colleagues watching automation move through radiology, pathology, and now oncology. Sometimes from patients who want to know whether the physician across from them is still the one making the decision.

I want to give a real answer. Not a reassuring answer designed to make everyone feel better. A real one — because I think physicians who pretend the question is ridiculous are not being honest, and physicians who catastrophize about obsolescence are not being accurate.

The truth, as I understand it from inside this field, is more nuanced and more interesting than either extreme.

What AI Does Well — and I Mean Really Well

Let me start by giving AI its due, because I think some of the defensiveness in medicine comes from a reluctance to acknowledge what the technology actually does. This has the potential to shape rural cancer care access.

AI-assisted auto-contouring in radiation oncology can reduce contouring time from hours to minutes in many cases — while maintaining clinically acceptable accuracy with physician oversight - approaching expert-level performance for many standard organs-at-risk (OARs) and common disease sites. That is remarkable. I say that as someone who has spent hours at a contouring workstation. The time saved is real, and the clinical implications for patient throughput and physician capacity are significant.

AI can identify imaging patterns and radiomic features that may correlate with treatment response, recurrence risk, or toxicity—though many of these applications are still being validated for routine clinical use.

AI can flag dosimetric anomalies in treatment plans before they reach the patient. In some health systems, AI models can help predict which patients are at risk of missing appointments. It can translate a clinical summary into a patient’s language in real time. AI-assisted workflows have the potential to shorten prior authorization timelines.

Most of these tools function within a ‘human-in-the-loop’ model, where physician oversight remains essential for validation and final decision-making.

These are not small things. These are capabilities that, deployed well, will improve the quality, safety, and reach of cancer care.

I believe that. I want to be on record believing that.

And Here Is What It Cannot Do

AI cannot walk into a room and know, before the patient says a word, that something has changed since last week.

That knowledge — the clinical intuition built from years of presence, from thousands of patient interactions, from the particular way a person holds themselves when they are in pain versus when they are afraid — is not a feature that can be engineered. It is learned. It is human. And in oncology, where patients are navigating one of the most frightening experiences of their lives, it matters.

AI cannot hold a family together in a consultation room while they absorb a terminal diagnosis. It cannot pace the conversation to what the patient can take in. It cannot detect that the patient’s daughter is crying while the patient is exerting strength, and adjust accordingly.

AI does not reliably navigate situations where guidelines are incomplete, conflicting, or inapplicable to the individual patient… and they don’t always fit. Oncology is full of patients who are the exception — whose comorbidities, whose prior treatment history, whose personal values place them outside the standard protocol. In those moments, clinical judgment is not a tiebreaker. It is the entire decision.

AI cannot assume clinical or legal accountability for a medical decision. When something goes wrong — and in medicine, things sometimes do go wrong — there must be a physician who can stand in that room, with that patient, and take responsibility. That accountability is not administrative. It is the foundation of the physician-patient relationship, and it is something no algorithm can assume.

And AI cannot build trust with a patient who has reason to distrust the system.

I think about this particularly in the context of communities that have been harmed by medicine historically — Black/African American patients, Indigenous patients, low-income patients who have been underserved or mistreated by the very institutions now asking them to interact with an AI-powered care management tool. The trust required for a patient to show up, to comply with treatment, to call when something is wrong — that trust is built person to person. Slowly. With consistency. With presence.

No algorithm builds that kind of trust on its own.

The Supervision Question Is Really a Values Question

I presented at the ACR Annual Meeting in 2024 on AI in rural radiation oncology. One of the most contested topics I addressed was supervision — specifically, whether AI-assisted remote oversight can maintain the standard of care in rural clinics that cannot recruit or retain a full-time radiation oncologist.

The debate is real. Some of my colleagues believe that any relaxation of direct in-person supervision requirements jeopardizes patient safety. I understand that position. I take patient safety with absolute seriousness. My training at AFRRI, my experience with RO-ILS incident data, my time opening clinics from the ground up — all of it has taught me how quickly things can go wrong and how important oversight structures are.

But I also know what happens to patient safety when there is no physician at all.

The patient who cannot access radiation treatment because the nearest clinic is 90+ miles away and the wait time is measured in weeks — his/her safety matters too. The veteran who doesn’t complete his cancer treatment because the logistics are impossible — his outcomes matter too.

The supervision question, framed purely as a clinical safety question, is incomplete. It is also a values question. Who do we focus on when creating oversight rules? Whose access do we consider standard, and whose do we view as unusual?

AI-assisted remote oversight, done with rigorous protocols, documented accountability, and a physician available for every critical decision, is not the same as no physician. It is a different model of physician presence — one designed for the reality of maldistributed specialty care, not the ideal of a fully staffed urban academic center.

I am not advocating for recklessness. I am advocating for honesty about what the current system already fails to provide.

What This Means for Physicians Asking the Question

If you are a clinician reading this and wondering about your relevance in a world where AI can contour, plan, predict, and flag — here is what I want to say to you directly:

The skills that AI is augmenting are the technical ones. The skills that remain irreplaceable are the human ones — presence, judgment, accountability, trust, and the ability to hold complexity that does not resolve into a data point.

If you are leaning into those skills — deepening your capacity for difficult conversations, your understanding of the patients who look nothing like the clinical trial populations, your ability to navigate the gray zones where guidelines fall short — you are not becoming obsolete.

You are becoming more essential.

The physician who understands both the technology and its limits — who can explain to a patient what the AI flagged and what it means, who can audit an algorithm’s recommendation with clinical judgment, who can ask the question the model didn’t know to ask — that physician is not replaceable.

That is the physician I am trying to be. Still learning. Still asking questions. Still convinced that the human element in medicine is not a sentimental attachment to the past.

It is the point.

One More Thing

I said at the outset that I wanted to give a real answer, not a reassuring one. So let me close with the part that is genuinely uncertain.

I do not know exactly what medicine will look like in twenty years. I do not think anyone does — including the people building the technology. What I know is that the decisions being made right now, about how AI is deployed, whose data it is trained on, who has access to it and who doesn’t, what oversight structures we require — those decisions will shape the answer.

Physicians are not passive recipients of that future. We are among the people who should be shaping it — asking the clinical questions, demanding equity in design, insisting on accountability, and staying in the room where these decisions are made.

That is not optional. That is our responsibility.

The patients who need us most are depending on us to get this right.

I want to start with a number, because I think numbers sometimes do what paragraphs cannot.

Black women are about 40% more likely to die from breast cancer than white women — even though incidence is not higher among Black women.

Not diagnosed more often. Not presenting with more aggressive disease at the same rate across all subtypes. Dying more often. From the same disease.

I have been a radiation oncologist for years. I have treated breast cancer patients. I know what guideline-concordant care looks like, and I know what it means when a patient doesn’t receive it. And when I look at that statistic, I do not see a biological mystery. I see a system that was not built with certain patients at the center — and the consequences of that failure, measured in lives.

Breast cancer is where the numbers are most visible in my field. But this disparity is not unique to oncology — it runs through heart disease, diabetes, maternal health, and nearly every major diagnosis. The fault lines are the same: race, income, geography, language. I speak to oncology because it is where I have spent my career. But make no mistake — we are describing a system-wide failure, not a disease-specific one.

I am a Black woman. I am an immigrant. I am Caribbean-born. Equity in medicine is not an abstract policy interest for me. It is personal in the way that only lived experience makes something personal. And it is professional in the way that only years of clinical practice can make you understand the distance between what the guidelines say and what a patient actually receives.

This piece is about that distance. And about what AI can — and cannot — do to close it.

The Data We Already Have

The disparity in cancer outcomes is not a new discovery. We have known about it for decades. We have published on it, presented on it, convened task forces about it, and written it into strategic plans.

And the gaps persist.

Breast cancer is where the numbers are most visible — but the pattern holds across every cancer type where radiation plays a role: lung, prostate, colorectal, cervical. The gap between who receives guideline-concordant care and who does not follows the same fault lines regardless of diagnosis. Consider what the data shows:

• Black women are about 40% more likely to die from breast cancer than white women — even though incidence is not higher among Black women

• Lower-income patients are less likely to receive guideline-concordant cancer treatment

• Rural patients often face longer travel times and greater barriers to radiation treatment centers — a real obstacle when treatment requires daily visits for multiple weeks

• And for millions of people in the United States with limited English proficiency, the barrier is not distance or income — it is language. Informed consent conducted in a language a patient does not fully understand is not informed consent.

These are not gaps waiting for data. We have the data. What we have lacked — consistently, across decades — is the organized will to act on it at scale.

I say this not to assign blame, but to name the actual problem. Because if we misdiagnose the problem, we will keep proposing the wrong solutions.

Where AI Enters the Conversation

I have been asked, in various forms, whether AI will fix health disparities. My honest answer is: it depends entirely on how we deploy it.

AI is not neutral. It reflects the data it was trained on, the priorities of the organizations that built it, and the questions its designers thought to ask. If those inputs are shaped by the same structural biases that produced our current disparities — and many of them are — then AI will not close the gap. It will encode it, accelerate it, and make it harder to see because it will be buried inside an algorithm that feels objective.

We already have examples of this. Pulse oximeters — not AI, but a technology assumed to be universal — overestimate oxygen saturation in patients with darker skin tones. During the COVID-19 pandemic, this bias contributed to delayed recognition of hypoxia in Black patients. A cardiac risk algorithm widely used in clinical practice was found to systematically underestimate cardiovascular risk in Black patients. The bias was not visible on the surface. It was embedded in the training data and the assumptions behind the model.

AI in oncology is not immune to this. If predictive models for cancer risk are trained predominantly on data from populations that were historically over-represented in clinical research — which they were — then those models will perform less accurately for everyone else.

This is not a hypothetical future risk. It is a present reality that requires present action.

What Intentional Deployment Looks Like

Here is the distinction I want to draw carefully:

When equity is built in from the start — not added as an afterthought — AI becomes one of the most powerful tools we have for addressing disparity at scale.

Predictive analytics for screening gaps.

AI can analyze claims data, demographic information, and social determinants of health to identify members who are overdue for

cancer screening and unlikely to self-initiate. Some health systems are already using these tools to identify patients at risk of food insecurity and other care barriers — enabling proactive outreach before those patients miss treatment. That is not theoretical. That is working now.

Natural language processing for communication barriers.

After-visit summaries translated in real time into a patient’s preferred language, adjusted to their health literacy level, with automated follow-up prompts when the next appointment hasn’t been scheduled. Early NLP-assisted communication tools and related interventions show promise for improving follow-up and care coordination. For a population where language has historically been the reason someone didn’t understand what they were supposed to do next — this matters.

Social determinants screening at scale.

Food insecurity. Transportation gaps. Housing instability. These are not soft factors. They are predictors of treatment completion, and in oncology, treatment completion is a predictor of survival. AI tools embedded in care management platforms can screen for these factors, match patients to community resources, and alert navigators when a high-barrier patient goes silent. The technology to do this exists. The decision to build it into standard workflow does not yet exist in most organizations.

The Guardrail Conversation Nobody Wants to Have

I want to be honest about the risk, because I think intellectual honesty is more useful than optimism.

AI deployed without diverse training data will produce biased outputs. AI that operates as a black box — where clinicians cannot see or explain its reasoning — erodes trust and creates accountability gaps. AI tools designed for smartphone interfaces exclude the patients who most need them, because digital access is itself inequitably distributed.

That is the equity paradox of AI in health care, and it requires us to ask hard questions before we deploy, not after.

What does your training data represent? Who was in the room when the model was designed? What happens to accuracy when the patient in front of you doesn’t look like the patient the algorithm was trained on? And who is accountable when the algorithm gets it wrong?

These are not questions that slow down progress. They are the questions that make progress worth having.

What I Believe

I believe the disparity gap in cancer care is closable. Not completely, not immediately, and not by technology alone — but meaningfully, measurably, within the professional lifetimes of the clinicians reading this.

I believe AI is one of the most powerful tools we have for closing it — if we build equity in from the start, require explainability and auditability, pair digital tools with analog outreach for patients without digital access, and keep physicians in the loop at every clinical decision point.

And I believe that the primary obstacle is not technical. It is organizational. It is the gap between knowing what the data shows and deciding — at the institutional level, at the payer level, at the health system level — that closing this gap is a priority worth resourcing.

We have had the data for decades.

The question has always been - the will.

On Service, Exposure & Evidence — An Oncologist Veteran-to-Veteran Perspective

I was enlisted for many years, but it wasn’t until I became an officer and approached retirement that I fully understood the importance of benefits, documentation, and maintaining a thorough written record.

As an enlisted soldier, I ETS’d without much thought about future benefits. That was my short-timer’s blind spot. I was ready to move forward, and I did not yet understand how much those records might matter later.

By default, I had my medical, dental, and training records — or at least copies — because we had to carry them to each new base assignment. But nobody sat me down and said,

I learned some things later, than I wish I had.

That is one reason I do this work now. Not to scare veterans. Not to overpromise. Not to make the VA claim process sound simple … when it is not.

I do this because many veterans were never taught how to organize their own story in a way that helps their records make sense.

Based on the medical record reviews I have done, many veterans either skip, rush, overload, or treat this form like an afterthought:

VA Form 21-4138, Statement in Support of Claim — often called a Personal Statement.

Depending on your claim and how you submit it, VA may direct you to use another supporting-statement form, including VA Form 21-10210 for lay or witness statements. Always confirm the current form with VA.gov, your VSO, accredited claims agent, or attorney before submitting anything.

But the principle is the same: your records matter. Your statement helps explain what those records may not clearly say.

What Is a VA Personal Statement?

A VA Personal Statement is your opportunity to explain, in your own words, what happened, when it started, how your symptoms developed, and how your condition affects your daily life.

Your service records may show where you served. Your medical records may show a diagnosis, treatment, imaging, medications, or procedures. But those records do not always explain the lived experience.

They may not show:

• Your MOS, duty, exposure, point of injury, or work environment

• When you first noticed symptoms

• How the condition changed over time

• What activities you stopped doing

• How symptoms (such as pain, fatigue, breathing problems, anxiety, numbness, weakness, or scars) may affect your day-to-day life

That is where a clear, factual Personal Statement can help.

It should not replace medical evidence. It should help organize the story around the evidence. The story around you.

The Personal Statement Is a Bridge

Think of your VA claim as three pieces:

1. Your service record — where you served, what you did, when you served, and what exposures, duties, events, or injuries may have occurred.

2. Your medical record — diagnoses, symptoms, test results, imaging, pathology, treatment notes, surgery reports, medications, and follow-up care.

3. Your lived experience — how this condition affects your body, your work, your sleep, your mobility, your relationships, your independence, and your daily life.

The Personal Statement helps connect those three pieces.

It does not need to be dramatic. It does not need to be long. It does not need to sound like a lawyer wrote it. It does not need to sound like AI wrote it.

It needs to be clear. It need to be in your voice. It need to be about you.

A strong statement helps answer:

• What happened?

• When did it start?

• How has it changed?

• How does it affect you now?

• What evidence supports what you are saying?

What Veterans Should Include in a VA Personal Statement

Here is a simple structure veterans may use when preparing a Personal Statement.

1. Identify the condition or issue

Start by naming the condition or symptom area you are discussing. Keep the opening simple.

• “I am submitting this statement in support of my claim for chronic back pain.”

• “I am submitting this statement in support of my claim related to respiratory symptoms after deployment.”

• “I am submitting this statement in support of my claim involving cancer and toxic exposure concerns.”

2. Explain the service event, duty, exposure, or injury

Be specific. Avoid vague phrases like: “I was exposed to a lot of STUFF.”

Instead, write what you know:

• Where you were stationed or deployed

• Approximate dates or timeframe

• Your MOS, duty, or work environment

• Known exposure concerns

• Injury, accident, event, or repeated physical stress

• Relevant deployment conditions

For toxic exposure or cancer-related claims, this may include burn pits, jet fuel, solvents, contaminated water, radiation, chemicals, occupational hazards, or other exposure concerns.

Do not exaggerate. Do not guess. Say what you know and what you personally experienced.

3. Describe when symptoms started and how they changed

A timeline matters. You do not have to remember every exact date, but you should try to give the clearest timeline you can.

Helpful questions:

• Did symptoms start during service?

• Did they begin soon after separation?

• Did they develop years later?

• Did they worsen over time?

• Did you seek medical care?

• Did you self-manage before finally getting evaluated?

Examples:

“I first noticed shortness of breath during deployment, especially after physical training. After returning home, I continued to have coughing episodes and gradually noticed I could not exercise the way I used to.”

Or:

“My pain began after repeated lifting and carrying during service. At first, I managed it with over-the-counter medication, but over time it became more frequent and began affecting my ability to sit, stand, and sleep.”

4. Describe daily impact in real-life terms

This is the section many veterans write too generally.

Instead of saying: “My knee hurts.”

Say:

Instead of saying: “I am tired.”

Say:

Concrete details help. Your Personal Statement should explain how the condition shows up in ordinary life.

Examples of daily impact:

• Trouble sleeping

• Difficulty walking, standing, sitting, lifting, bending, or climbing stairs

• Needing help with household tasks

• Avoiding certain activities

• Missing work or changing work duties

• Pain with movement

• Fatigue/Tiredness after treatment

• Shortness of breath

• Neuropathy, numbness, weakness, or balance issues

• Scars, skin changes, or surgical complications

Do not pack every single symptom into your statement. Stay factual. Stay specific.

5. Point to supporting evidence

Do not assume the reviewer will automatically know which records matter most. If you have supporting records, point to them.

Examples:

• Private medical records

• VA treatment notes

• Imaging reports

• Oncology/cancer records

• Radiation therapy summaries

• Medication lists

• Buddy statements

You might write:

“My oncology (cancer) records from March 2024 show the diagnosis and treatment plan.”

Or:

“My post-operative follow-up notes describe ongoing pain, scar sensitivity, and limitations.”

The goal is to help the record tell a clearer story.

What Veterans Should Avoid in a Personal Statement

A Personal Statement should be honest, organized, and useful. Here are a few things to avoid.

Avoid self-diagnosing

Describe what you experienced. Let medical records and qualified medical professionals address diagnosis.

Instead of: “I know my cancer was caused by burn pits.”

Consider: “I served near burn pit exposure during deployment. I was later diagnosed with cancer. I am submitting this statement to explain my service history, exposure concerns, diagnosis, and treatment records.”

That keeps the statement factual.

Avoid rambling

One statement should usually focus on one condition or one connected issue. If you are discussing several unrelated conditions, consider organizing them separately so each issue is clear.

The reviewer should not have to dig through pages of unrelated information to find your point.

Avoid using the statement only to vent

The VA process can be frustrating. Many veterans have every right to feel tired, dismissed, delayed, or confused.

But the Personal Statement is not the best place to unload that frustration.

Keep it focused on:

• What happened

• What you experienced

• How it affects you

• What evidence supports it

That is how you make the statement useful.

Avoid copying generic language from the internet

Templates can help with structure, but your statement should sound like your life.

Your facts matter. Your timeline matters. Your symptoms matter. Your medical records matter.

Use a template if needed, but do not let a template erase your actual story.

A Simple VA Personal Statement Framework

Here is a plain-language structure you can follow:

Opening: “I am submitting this statement in support of my claim for [condition].”

Service connection or exposure: “During my service, I [describe event, duty, injury, exposure, location, or timeframe].”

Symptoms and timeline: “I first noticed [symptoms] around [timeframe]. Since then, [describe how symptoms changed].”

Daily impact: “This affects my daily life by [specific examples].”

Evidence: “My records from [date/provider/type of record] support this, including [brief description].”

Closing: “I am providing this statement to help explain my symptoms, timeline, daily limitations, and supporting records.”

That is enough.

Clear is better than dramatic.

Organized is better than long.

Specific is better than emotional.

Why This Matters for Cancer, Toxic Exposure, and Complex Medical Claims

For veterans with cancer, toxic exposure concerns, or complicated medical histories, the record can become difficult to follow.

There may be:

• Service records

• Deployment history

• Exposure concerns

• Civilian medical records

• VA medical records

• Oncology consultations

• Surgery notes

• Pathology reports

• Radiation treatment summaries

• Chemotherapy records

• Survivorship follow-up notes

• Late effects or residual symptoms

That is a lot. And in a language that is dense for many - Medical Lingo - can quickly become overwhelming.

A Personal Statement can help explain how the pieces fit together from the veteran’s point of view.

Again, the statement does not replace medical evidence. It helps point to it.

Veteran to Veteran: Do Not Treat This Like a Small Form

I started my military career as an enlisted Soldier and later retired as an Army Lieutenant Colonel. I am also a physician with a specialty in cancer care - Radiation Oncology.

I know what it feels like to find out late that there were things I should have been taught earlier.

That is why I am saying this plainly:

• Do not treat your Personal Statement like a throwaway form.

• Do not rush it.

• Do not turn it into a novel.

• Do not use it to diagnose yourself.

• Use it to tell the truth clearly.

Your service record may show where you were.

Your medical record may show what was diagnosed.

Your Personal Statement helps explain how it affected you.

That matters.

Why This Question Keeps Coming Up

Since the PACT Act passed, one question comes up more than almost any other — from veterans, from spouses, from adult children filing on behalf of a parent:

“If my condition is presumptive and I was there during the qualifying period, is my VA disability rating automatically granted?”

It is a reasonable question. An understandable assumption. And one of the most important misunderstandings I see in VA claims work. Learn the basics about the PACT Act HERE.

The short answer: Presumptive does not mean automatic.

Let me explain why — and what the VA actually still needs from you.

What “Presumptive” Actually Means (In VA Terms)

In VA language, presumptive means the VA agrees not to require proof that your military service caused your condition — as long as you meet:

1. specific location

2. specific timeframe, and

3. specific diagnosis criteria.

That is an important legal shortcut. It removes the causation debate. But it is only one step in the process.

The VA still must confirm you have a qualifying diagnosis, determine whether the condition is active or producing ongoing residual effects, and apply rating criteria outlined in federal regulation.

Presumptive status removes the need to prove why you got sick. It does not remove the need for medical documentation about how sick you are.

Being “There” Is Necessary — But Not Sufficient

Many veterans stop at “I was there.” They confirm their deployment on their DD-214, confirm the timeframe, and assume the claim is complete. Presence in a qualifying area is essential. But it is not the finish line.

The VA still evaluates:

• Whether your medical condition meets diagnostic standards

• Whether it is currently active, in remission, or producing residual effects

• How severe the condition is under VA rating criteria

Service connection and disability rating are two separate decisions. You can receive service connection and still have a claim stall or come back lower than expected — because the medical picture wasn’t documented clearly enough.

What the VA Still Needs — Even for Presumptive Claims

1. A Confirmed Diagnosis

The VA requires medical evidence of a diagnosis that meets accepted standards. For cancer: pathology or biopsy reports, oncology notes, imaging and lab results, and treatment summaries.

2. Evidence of Current Status

Is the condition active, in remission, or stable with ongoing residuals? This distinction directly affects rating. Active cancer is often rated at 100%. Post-treatment status may be rated on residual symptoms — not the original diagnosis.

3. Information Needed for Rating

Even when service connection is established, the VA must assign a percentage. That requires severity indicators, functional impact, treatment history, and residual complications. This is where most claims stall — not eligibility, but documentation.

Where Nexus Letters and DBQs Fit In

For presumptive conditions, a Nexus Letter is often unnecessary for service connection. The law has already established that link.

However, medical opinions and DBQs can still be helpful when the diagnosis is complex or atypical, the condition is no longer active but has significant ongoing residuals, or there is disagreement about recurrence, progression, or severity.

Used appropriately, these tools support clarity. Used indiscriminately, they can create confusion — and unnecessary expense.

Why Presumptive Claims Still Get Delayed or Denied

Common reasons include missing medical records, outdated documentation, misunderstanding of rating criteria, and lack of clarity about disease status.

A denial does not always mean the veteran is ineligible. Often, it means the VA did not have enough information to rate the claim correctly.

Clarity does not weaken a claim. It strengthens it.

The Bottom Line: Presumptive Is a Doorway, Not the Finish Line

The PACT Act represents real, meaningful progress for veterans exposed to toxic hazards. But presumptive status is a starting point — not a guarantee.

Veterans who understand what the VA still requires are better positioned to advocate effectively, avoid unnecessary delays, and reduce frustration. Education is one of the most powerful tools in this process. Need assistance? Schedule a VA Case Strategy Consult.

It is not done.

Your surgeon needs to see you after treatment — not because something is wrong, but because they are watching for things your oncologist cannot always see: how the surgical site is healing, whether a reconstruction is holding, and early signs of local recurrence right where they operated.

Friday Food for Thought

Where have I confused loyalty with obedience?

MUSINGS: What have I brought with me into this season that God never actually told me to carry? And am I obeying His instruction — or editing it to protect what I am afraid to release?

The Original Directive — Genesis 12:1

God told Abram: “Go from your country, your people, and your father’s household.”

Three things. Country. People. Father’s household.

Lot was his nephew — son of his deceased brother Haran. So in one sense, Lot was part of the household. And yet — nowhere in the text does God say “bring Lot.” Genesis 12:4 simply says “Lot went with him.” No divine instruction attached to that.

That distinction matters a lot in the obedience conversation.

The Problems That Followed

Once Abram took Lot, a chain of complications followed:

• Genesis 13 — Their combined flocks and herds grew so large there wasn’t enough land. Strife broke out between their herdsmen. It forced a separation.

• Lot chose Sodom — He picked the lush, well-watered plain toward Sodom, a city described almost immediately as exceedingly wicked. That choice said something about Lot’s discernment.

• Genesis 14 — Because Lot was in Sodom, he got caught up in a regional war and was captured. Abram had to mobilize 318 trained men to go rescue him. That’s now Abram’s problem — a problem that originated from a person he wasn’t told to bring.

• Genesis 18-19 — When God reveals He’s about to destroy Sodom, Abraham intercedes specifically because Lot is there. He’s now negotiating with God partly over the consequences of a decision he made back in chapter 12.

• Lot’s rescue and aftermath — The angels had to practically drag Lot’s family out. His wife looked back. His daughters, traumatized and isolated, made decisions that produced Moab and Ammon — two nations that would be thorns in Israel’s side for generations.

The Obedience Analysis

The theological tension here is this:

God said leave your people and your father’s household. Lot represented a thread of connection to exactly what God said to release. Abram didn’t defy God — but he edited the directive. And edited obedience has a long track record of producing complicated fruit.

Some scholars and teachers land here:

Others argue:

Both interpretations have merit. But what’s hard to argue against is the consequence trail. Whether or not Lot was “wrong” to bring, the presence of Lot created repeated interruptions, diversions, and heartache in Abram’s journey.

The Principle Worth Sitting With

“Who or what am I still carrying simply because letting go feels too costly?”

Sometimes what God doesn’t mention in a directive is just as significant as what He does. When He says go — He may already know what you need to leave behind in order to go fully. The people, attachments, and loyalties we add to the assignment can become the very complications that slow or complicate the promise.

Abram still became Abraham. God’s plan was not stopped. But Lot’s story is a long, complicated, painful one — and much of it traces back to a single unasked-for addition to an otherwise clear call.

In those situations, the question often becomes whether the medical record, service history, exposure history, and diagnosis can reasonably support a connection.

That is where a Nexus letter may be helpful. Whether Air Force, Navy, Army, Coast Guard, Reserve, or National Guard, certain military jobs have direct or repeated exposure risks. Toxic exposure may open the door, but your medical records help tell the story.

Hearing that you may need radiation treatment can stir up all kinds of emotions.

Even strong, educated, capable people — the ones who manage households, lead teams, care for others, and keep life moving — can feel suddenly unsure when cancer enters the conversation.

And when the word radiation comes up, it can sound scary, mysterious, or more intense than it really is.

Over the years, I have cared for patients in both military and civilian settings, and one thing I have seen again and again is this:

Fear often grows in the space where clear information is missing.

Your first radiation visit is not just about machines, schedules, or medical terms. It is a chance to better understand your treatment, ask honest questions, and begin feeling more grounded about what comes next.

So if you are heading into that first visit feeling nervous, here are some questions I want you to know you are allowed to ask.

First, take a deep breath

Your first radiation oncology visit is usually a consultation, not your first treatment day.

That visit is often focused on:

• reviewing your diagnosis and medical history

• discussing why radiation is being recommended

• talking about benefits, side effects, and timing

• answering your questions

• helping you understand the road ahead

You do not have to walk in knowing all the right words.

You do not have to impress anyone.

You do not have to pretend you are less afraid than you are.

You simply need to come ready to listen, ask, and learn.

Questions to ask at your first radiation visit

1. Why do I need radiation?

This is one of the most important questions.

Radiation is not recommended for every cancer or every person in the same way. Ask your doctor to explain why radiation is being advised in your specific case.

You may want to ask:

• Is radiation being used to reduce the risk of cancer coming back?

• Is it being recommended after surgery?

• Is it being used to treat a known area of disease?

• Is it being used to help with symptoms?

Knowing the purpose of treatment can ease a lot of anxiety.

2. What is the goal of my treatment?

This question sounds simple, but it matters.

Is the goal cure, control, or symptom relief?

When you understand the goal, treatment may feel less like something that is just being done to you and more like a thoughtful part of your overall plan.

3. How many treatments will I need?

Not every radiation plan looks the same.

Some people need only a few treatments. Others may need several weeks. Ask:

• How many treatments are planned?

• How often will I come in?

• How long does each visit usually take?

• When will treatment likely start?

This helps you prepare emotionally, physically, and practically.

4. What side effects should I expect during treatment?

Please ask this plainly.

You deserve a realistic answer, not a rushed summary.

Side effects depend on the area being treated. Treatment to the breast is different from treatment to the pelvis, lung, brain, prostate, head and neck area, or another part of the body.

Depending on your treatment area, side effects may include fatigue, skin irritation, soreness, swallowing changes, bowel or bladder symptoms, or other concerns.

Ask your team:

• What side effects are common?

• What side effects are less common but important to know?

• What symptoms should make me call the office?

5. What side effects could show up later?

This question is just as important as asking about the short-term effects.

Some side effects improve quickly after treatment, while others may take longer to settle or can appear later after the treatment is completed. That does not mean you should panic. It simply means you should understand the full picture. Knowing what to expect will take away the surprise factor. If you are prepared, then you will know what to do. Some patients may not notice any side effects from treatments.

However, I want you to know that asking about long-term effects is not being negative. It is being informed.

6. What can I do to take care of myself during treatment?

This is such a helpful question because it moves you from fear into action.

Ask your radiation oncologist and care team what they recommend for:

• skin care

• fatigue

• exercise

• hydration

• nutrition

• clothing

• products to avoid

• daily routines

You do not have to figure everything out by yourself. Your team should help guide you.

7. Will treatment affect my work, energy, or daily routine?

Many people are trying to balance cancer treatment with work, caregiving, appointments, travel, church, family responsibilities, and everyday life.

Ask what you can realistically expect.

Some patients do very well during treatment and continue most of their usual routine. Others need more rest, more support, or schedule adjustments.

There is no prize for pushing yourself too hard.

Plan ahead where you can. Accept help where you need it. And give yourself grace.

8. What symptoms should make me call you/office immediately?

Do not leave this part vague.

Ask:

• What is considered expected?

• What would be concerning?

• Who do I call during office hours?

• Who do I call after hours, if needed?

Knowing when and how to reach out can reduce panic and help you feel more secure.

9. Are there long-term quality-of-life concerns I should know about?

This question matters, especially when you are thinking beyond treatment and into survivorship.

Depending on the area treated, you may want to ask about:

• skin changes

• scarring or fibrosis

• swelling or lymphedema

• bowel or bladder changes

• sexual health

• fertility concerns

• menopause symptoms

• fatigue

• pain or stiffness

Not every person will experience these things, but they are worth discussing.

Your quality of life matters too.

10. What follow-up happens after radiation is over?

Many patients are surprised by how emotional the end of treatment can be.

You spend time preparing, showing up, adjusting your routine, and doing what needs to be done. Then one day, treatment ends.

That can bring relief.

But it can also bring uncertainty.

Ask:

• When will my follow-up visits happen?

• What symptoms should I keep monitoring?

• What should recovery look like?

• Which doctor should I contact for different concerns?

• Will I need scans, labs, or exams after treatment?

Survivorship is part of the care plan, not an afterthought.

A few practical things to bring with you

You may be experiencing symptoms. Be sure to make a note of duration of symptoms, location, etc., If possible, bring:

• a trusted support person, if allowed and helpful

• a notebook or notes app [see our BREAST Cancer Journal HERE and Prostate Cancer Guide HERE]

• a list of your questions

• your medication list

• a list of symptoms you are having, including when they started and where they are located

And please do not be embarrassed to write things down.

Anxiety can make even simple information harder to remember. That is normal. Write it down, ask again, and take your time.

One more thing I want you to remember…

You are allowed to ask for an explanation again.

You are allowed to say, “I don’t understand.”

You are allowed to ask, “Can you explain that in plain language?”

You are allowed to ask, “Can you show me a picture?”

You are allowed to pause and write things down.

You are allowed to advocate for yourself.

This is your body.

This is your care.

This is your life.

Being informed does not take away every fear, but it can make the path feel less lonely and more manageable.

Let’s be real. When I first enlisted, I did not know a thing about VA disability benefits.

I was young, strong, and ready for the next assignment. I did not think I would get hurt. I did not think I would get sick. And I definitely did not understand why medical documentation would matter later.

But life happens. Injuries happen. Illness happens. Sometimes symptoms show up while you are still serving. Sometimes they follow you home. And too many service members leave the military without the knowledge they need to protect their health, their family, and their future.

This post is for you - whether you are still on active duty, preparing to transition, retiring after a long career, or already living life as a veteran.

Not to teach you how to game the system. Not at all.

This is about understanding your body, knowing your medical story, and learning how to present the truth clearly.

1. Start Before You Leave the Military

If you are still serving, please hear me: your exit strategy should not begin the week you clear post.

I know the short-timer attitude can get thick. You are counting days, thinking about the next chapter, and trying to finish the mission. But this is exactly when your medical documentation matters.

The VA has a pre-discharge process called Benefits Delivery at Discharge, or BDD. In general, eligible service members with a known separation date may file a disability claim 180 to 90 days before leaving active duty. The goal is to help VA review records, schedule exams, and move the claim along before separation.

That 180-day window matters. Do not wait until the last minute if you can help it.

Active-duty takeaway: If something hurts, changed, worsened, or requires treatment, get it documented while you are still in uniform. Medical records are not just paperwork. 2. What Is VA Disability Compensation?

VA disability compensation is a tax-free monthly payment for veterans with disabilities that were caused or made worse by military service.

It is not a handout. It is not a reward. It is compensation for service-connected conditions that affect your health and daily life.

VA disability ratings generally range from 0% to 100%. A 0% rating may not pay monthly compensation, but it can still matter because VA has recognized that the condition is service-connected. Higher ratings are based on how much the condition affects function, earning capacity, and daily life according to VA criteria.

Plain language version: The question is not simply, “Do I have a diagnosis?” The bigger question is, “Is there a service connection, and how does this condition affect my life?” See the VA-link below for more information.

3. Understand Service Connection

For a VA disability claim, the medical issue usually needs to connect back to your military service. That connection may be direct, aggravated by service, secondary to another service-connected condition, or presumed because of certain exposures or circumstances.

This can include many types of conditions, such as:

• musculoskeletal problems like back, neck, knee, shoulder, or hip conditions

• hearing loss or tinnitus

• mental health conditions such as PTSD, anxiety, or depression

• respiratory problems, including asthma or chronic breathing concerns

• skin conditions, gastrointestinal issues, headaches, or sleep problems

• conditions related to toxic exposures, depending on your service history and VA rules

4. Know Your Body and Your Diagnosis

This is where my physician brain and military brain come together.

You do not need to become a doctor. But you do need to know your own body.

This kind of detail helps your medical team document your condition clearly. It also helps you explain your limitations without exaggeration and without minimizing what is real.

Nothing more. Nothing less. Just the truth, clearly explained.

5. Medical Records Matter More Than Memory

Memory is helpful, but records carry weight.

Start gathering and organizing:

• service treatment records

• civilian medical records

• medication lists

• imaging reports and lab results

• specialty clinic notes

• physical therapy or occupational therapy notes

• deployment, exposure, or incident-related documentation when available

• personal notes about symptoms, frequency, and impact on daily life

Do not assume everything is easy to find later. Keep your own copies whenever possible. Scan documents. Save PDFs. Use a folder system that makes sense to you.

6. Work With Your Healthcare Team

Your doctors, therapists, and clinicians are not your enemies. Most are trying to understand what is going on and document it appropriately.

But they cannot document what you do not tell them.

Be honest about pain, function, sleep, mood, limitations, flare-ups, medications, missed work, and how your condition affects daily life. If something is embarrassing, say it anyway. If something is getting worse, do not brush it off just because you are used to pushing through.

Military culture often teaches us to keep moving. That may help with the mission, but it can hurt you medically if you never report symptoms until years later.

A simple phrase to use: “I want to make sure this is documented clearly because it has been affecting my daily life.”

7. After You File, Do Not Miss the Exams

After you file a VA disability claim, VA may schedule a claim exam, often called a C&P exam. The exam helps VA evaluate your claimed condition and how it affects you.

Please do not miss these exams. If you cannot attend, contact VA or the contractor as instructed and reschedule as soon as possible.

Before the exam, review your symptoms and history. During the exam, be truthful and specific. Do not perform for the examiner, and do not minimize because you are having a good day. 8. Ask for Help Early

VA-accredited representatives, Veterans Service Organizations, and trained benefits counselors can help you understand the process, prepare paperwork, and avoid common mistakes. If you are still serving, also use your transition resources and benefits briefings.

Ask questions. Take notes. Keep copies. And if someone explains something in a way that does not make sense, ask them to explain it again in plain language.

Even the most capable person can feel nervous sitting in an exam room, waiting for results, trying to remember symptoms, medications, dates, and questions.

That is why preparation matters.

Not because you need to impress your doctor.

Not because you need to know medical language.

But because this is your body, your health, your time, and your life.

BEFORE THE VISIT: prepare without panic

You do not need a perfect script. You simply need a few notes that help your care team understand what is happening.

1. Write down your top concerns

Before the visit, write down the main reason you are going.

Try to include:

• what you are feeling

• when it started

• where it is located

• what makes it better

• what makes it worse

• what you have already tried

For example:

“My lower back started hurting last week after gardening. It felt sharp at first, then tight. Bending makes it worse. Rest and Tylenol helped for a few hours, but the pain keeps coming back.”

That kind of information helps your doctor help you.

2. Bring your questions

Please do not wait until the doctor is walking out of the room to remember your biggest concern.

Write your questions down before the visit.

You may want to ask:

• What do you think is causing this?

• What are we checking for?

• What are my options?

• What symptoms should make me call back?

• When should I expect results?

• What should I do if things get worse?

If you are there for a routine visit, you can ask:

• What screenings do I need at my age?

• Are my vaccines up to date?

• Are there changes in my labs I should understand?

• Is there anything in my family history that changes my risk?

3. Know your medications

Bring a current medication list or take pictures of your bottles.

Include:

• prescription medications

• over-the-counter medications

• vitamins

• supplements

• herbal products

• allergies and what reaction you had

This matters. Supplements, teas, and over-the-counter medications can still affect bleeding risk, blood pressure, blood sugar, sedation, procedures, and prescription medication safety.

4. Bring support when the appointment is important

For routine visits, you may be fine going alone.

But for major results, cancer visits, heart concerns, surgery discussions, new diagnoses, procedures, or anything that feels emotionally heavy, bring someone if possible.

A trusted person can help you listen, take notes, ask questions, and remember what was said.

That is not weakness.

That is wisdom.

DURING THE VISIT: speak up early

Once you are in the room, it is easy to get pulled into the flow of the appointment.

The nurse asks questions. The doctor comes in. The computer opens. Time moves quickly.

So speak up early.

You can say:

“Before we start, I wrote down three things I really want to make sure we discuss today.”

That helps focus the visit.

If you wait until the end, there may not be enough time to address what matters most.

1. Ask for plain language

You are allowed to say:

“Can you explain that in plain language?”

You are allowed to ask:

“What does that mean for me?”

You are allowed to say:

“I want to make sure I understood you correctly.”

That is not being difficult.

That is being responsible.

2. Be honest, even when it feels uncomfortable

Your doctor needs accurate information to help you.

Please be honest about:

• alcohol use

• smoking

• supplements

• missed medications

• pain

• bowel or bladder symptoms

• sexual health concerns

• mood changes

• falls

• memory concerns

• financial or transportation barriers

This is not about shame.

It is about safety.

Your care team cannot help with what they do not know.

3. Before you leave: get the plan

This part is important.

Do not leave the visit with only a general feeling that “they said something about labs” or “I think I’m supposed to follow up.”

Before you leave, ask for the plan.

You can say:

“Let me repeat this back to make sure I have it right.”

Then confirm:

• What diagnosis or concern was discussed?

• What medication was started, stopped, or changed?

• What labs, scans, or referrals are needed?

• When should I expect results?

• Who will contact me?

• When should I follow up?

• What symptoms should make me call?

This one habit can prevent so much confusion.

AFTER THE VISIT: do not disappear from your own care

The appointment may be over, but your care is still moving.

After the visit:

• review your notes

• schedule follow-up appointments

• complete labs or imaging as instructed

• pick up medications

• read after-visit instructions

• check your patient portal

• call if something does not make sense

And if you see an abnormal result in the portal before anyone calls you, do not panic — but do follow up.

You can call or message the office and ask:

“I saw this result in my portal. Can someone explain what it means and what I should do next?”

That is a reasonable question.

One more thing I want you to remember

You do not need to become a medical expert before your appointment.

You simply need to come prepared enough to participate in the conversation.

• Write it down.

• Bring the list.

• Ask the question.

• Tell the truth.

• Repeat the plan.

• Follow up.

Your doctor is there to help, but you are not a passive passenger in your care.

You are part of the team.

And when you understand the plan, you are more likely to follow it, question it when needed, and advocate for yourself with confidence.

Taking care of yourself can feel like a full-time job. I understand that.

But this one body is the only one you have.

So take the note.

Ask for plain language.

Bring support when needed.
